Supervisor Appointment Form
This form is for a site’s director of training to notify Fuller of those that are supervising Fuller students on a yearly basis.
The Supervisor Appointment form needs to be completed and CVs of supervisors of Fuller’s doctoral-level clinical psychology students must be attached.
Once the Supervisor Appointment form has been completed, fill in the required fields and then select “Choose File” for the Appointment Form. Choose the file from your device. Select “Choose Files” next to Upload CVs. Select all the CVs that need to be uploaded. This is easiest if all the CVs are gathered into the same folder. To select multiple files, hold down the “command” key on a Mac or “Ctrl” key on a Windows PC.
