Please note:

  • All students will get a diploma upon graduation. This form is used to order additional, replacement or alternative diplomas
  • You may request multiple copies of a diploma
  • If you have multiple degrees, you will need to fill out this form separately for each degree
  • The cost for each diploma requested is $25.
